COVID-19 Testing Sites

Hello JUSD parents:


We are now well into our school year and are very excited to have everyone back on campus. We also understand the concerns you may have about COVID-19. JUSD is committed to providing a safe learning environment for all students. In an effort to help stop the spread of the virus, we will be offering free COVID-19 testing for all students and staff, regardless of their vaccination status or whether they have insurance.

We are offering this testing opportunity (in partnership with PMH Laboratory Inc.) at schools throughout the district. A schedule and more information, including forms you will need to complete are available on this web page. Please see the attached flyers for more information.

Registration for testing is available online at: https://bit.ly/PMHLAB. Participants will need an access code to register. The access codes are site-specific and are available here on our website (Rustic Lane Access Code: 1876). We are asking participants to use the access code for their child’s school site. However, once registered, participants will be able to go to any location to test. Parents/guardians can test at the Education Center, 4850 Pedley Road.

More detailed information about how to fill out the form also is available here. Please note that while the registration form includes a space for social security number, participants should NOT include their social security number. 

Please also note that those getting tested will be asked for an insurance card and identification; however, no one will be turned away. Those without insurance will be asked to fill out an uninsured patient form (which is available to download here).

Those who register for testing also will need to complete and bring with them:
Test results will be provided via email within 24-48 hours to the email address provided on the registration form. A positive result will be communicated with a phone call.
