Case Management

We provide case management supports in connecting families with "self-sufficiency" resources and provide support to complete application forms.

Referral Sources

By managing agency contracts, we manage a "virtual" behavioral health center.  We utilize a common referral form for all agencies, determine the best immediate referral source, and connect the family with the agency.  Additionally, we meet and consult with our providers on process and supports.  Through our various contracts we have expanded mental health to the extent of over $500,000 in services provided directly to students and families.

School Site Supports: Social Worker Interns & Outreach Workers

We work with local universities such as USC, La Sierra, CSU San Bernardino, Cal Baptist University, Arizona State, CSU Fullerton, and Loma Linda to bring social work interns, BSW & MSW.  These interns are placed at participating schools to provide immediate and direct support to sites by providing low level counseling services, supporting case management services, and providing behavioral health referrals.  This is a site based service based on site LCAP funding for supervision and availability of interns.
